What is the waist-to-height ratio?

WHtR = waist circumference ÷ height, in the same units. A 80 cm waist at 170 cm height gives 0.47. The ratio works because it captures central fat — the abdominal, largely visceral fat most linked to metabolic disease — and automatically scales it to body frame. It needs no sex-specific cutoffs, no age tables and no scale: one tape, one division.

The 0.5 rule

The evidence converges on a beautifully simple boundary: health risk rises measurably once your waist passes half your height. The rule is robust across sexes and ethnic groups, and UK national guidance (NICE) formally adopted it in 2022, recommending adults keep waist-to-height below 0.5. In head-to-head studies, WHtR predicts type 2 diabetes and cardiovascular risk at least as well as — and frequently better than — BMI, because it sees the dangerous fat that BMI averages away.

How to measure your waist

Find the midpoint between your lowest rib and the top of your hip bone — for most people at or just above the navel. Tape horizontal, snug but not digging in, arms relaxed, measured at the end of a normal exhale. Measure twice and average. Height is measured without shoes. Small technique errors matter here more than in most metrics, because the healthy band is only about 10 cm wide.

WHtR vs BMI and the other body metrics

BMI cannot tell a muscular torso from a fat one, and it misses “normal-weight central obesity” — people of healthy weight carrying risky abdominal fat, a group WHtR flags reliably. Against the waist-to-hip ratio, WHtR is simpler (one measurement, one universal cutoff) and at least as predictive in most studies. If your ratio is 0.5 or above

Central fat is the most responsive fat: a moderate calorie deficit, regular aerobic plus resistance exercise, better sleep and less alcohol shrink the waist before anything else. Even 3–5 cm off the waist meaningfully improves the metabolic numbers that ride along with it — blood pressure, triglycerides, blood sugar. A persistently high ratio alongside other risk factors (family history, high blood pressure, abnormal lipids) is a good reason for a medical check-up rather than another app.
