How to measure waist and hips correctly

The ratio is only as good as the tape work. WHO protocol:

  • Waist: stand relaxed, find the midpoint between your lowest rib and the top of your hip bone (roughly at or just above the navel), wrap the tape horizontally, snug but not compressing, and measure at the end of a normal exhale.
  • Hips: tape around the widest part of your buttocks, feet together, tape level all the way round.

Measure directly on skin or thin clothing, twice, and take the average. Then: WHR = waist ÷ hip. A 80 cm waist over 100 cm hips is a ratio of 0.80.

The sex difference is real biology: estrogen steers fat toward hips and thighs, so healthy cutoffs sit lower for women.

Apple vs pear: why the ratio predicts risk

A high WHR means proportionally more fat around the abdomen — and abdominal fat is disproportionately visceral fat, the metabolically active kind wrapped around the organs. Visceral fat drives insulin resistance and inflammation, which is why an elevated WHR associates with type 2 diabetes, high blood pressure and cardiovascular disease even at a “normal” body weight. Hip-and-thigh fat, by contrast, is mostly subcutaneous and metabolically quieter — the pear shape genuinely carries less risk than the apple at the same weight.
