How quickly can ALT levels change?

Fairly quickly. ALT has a half-life in blood of roughly 47 hours, meaning levels can fall by about half every two days once the source of liver stress is removed. Elevations from alcohol or exercise often normalize within one to two weeks. Improvements from weight loss in fatty liver take longer — typically weeks to months of sustained change — which is why follow-up tests are usually scheduled at least several weeks apart.
