Advanced cirrhosis, especially from long-standing hepatitis C or fatty liver, can present with normal or only slightly raised enzymes because few working cells remain to leak them. Albumin, bilirubin, platelet count and clotting tests speak more directly to liver function.
What does SGOT and SGPT indicate together?
Read together, SGOT (AST) and SGPT (ALT) indicate whether the liver is the likely source of injury and suggest the pattern behind it. ALT is concentrated in the liver, so a rise in both with ALT higher points toward liver causes such as fatty liver or viral hepatitis. AST higher than ALT raises the question of alcohol, advanced scarring or a muscle source. Their relative levels and trend over time guide the next tests.
How long does it take for SGOT to return to normal?
It depends on the cause. AST has a half-life of about 17 hours, so once an injury stops the enzyme clears within days; exercise-related rises typically normalize within a week. Alcohol-related elevations usually improve within two to four weeks of stopping and may normalize in one to three months. Fatty liver improvements take months of sustained change. Persistent elevation despite removing suspected causes warrants further investigation.
